Sword Master and Master of Horses are not "ranks" in the military sense, but offices. A Duke will have them for the purposes of training men-at-arms and millitia, and cavalry. They are members of his staff and carry a rank similar to Housecarl, Reeve, etc.
Fannon was in an unusual situation because not only was he Sword Master, he was Sword Master for a Duke who also held Kingdom Rank.
